I authorize Alpin Haus to charge me if the ski package is not returned by April 1^st for the amount of $250. I accept full financial responsibility for the equipment listed on the agreement. Rentals include ski/board, boots and bindings.

I promise to return it without any additional stickers other than those provided by Alpin Haus, cleaned, undamaged, and by the agreed time and date, and if I fail to do so I will pay for its repair cleaning or replacement at the full retail rate, as determined by the shop, as well as for the full rental value of any additional days.

Due to high demand of lease equipment, if not picked up within 30 days of original date the equipment will be released back into inventory, and store credit will be issued.

I understand how this equipment works and have received instructions and satisfactory answers to any questions.

I agree to check this equipment before each use and if at any time this equipment does not seem to be working properly, I will stop using it and immediately return it for inspection and possible repair/adjustment.

If this equipment is to be used by someone other than me, I certify that I am acting as agent for the other user and that I will provide this form and all pertinent warnings and information to the user.

I understand the risks and hazards related to skiing/snowboarding and I assume all liability of risks and hazards related to skiing/ snowboarding. I certify that all weight, height, age, and ability level provided are accurate.
